WHAT TO EXPECT

At your first visit, your therapist will take a thorough health history and do a comprehensive evaluation, then devise a custom treatment plan for you.

Subsequent visits will focus on strengthening and training specific muscle groups to promote balance and healing. Your therapist may use manual therapy, taping or other modalities to promote healing.

Your therapist will assess your progress, and our athletic trainers will assist you in executing your exercise plan.

 

Visits last 1 hour. Appointments start on the hour and 1/2 hour.

Please arrive on time.

Please allow time to find street parking. There is a lot of construction and road closures going on around us which can require extra time.

INSURANCE BENEFITS/PAYMENT

We will make every effort to understand your insurance benefits, however a quote of benefits is not a guarantee of payment. Please contact the customer service number on the back of your insurance card to verify your benefits for physical therapy.

 

Please allow 4-6 weeks for insurance to process your claim.

Once your claim has processed, you will receive a monthly billing statement by mail.

We accept credit/debit/HSA cards by phone, mail, in the clinic, or online at www.myproviderlink.com using your account number and form ID on your billing statement.

Payment of Co-Pays and Out of Pocket charges are due at the time of service.

For patients paying out of pocket, we charge $120 per visit.
